– Name of photo(s) you wish to use
– Context in which the photo(s) will be used (i.e. what type of book, magazine, website)
– Name of the book, magazine or website
– Duration of photo use
– Name and address of your company
– Contact person name, e-mail and phone number
– Any questions you may have
